1. Appointment
The Clinic appoints the Prescriber as an Independent Nurse Prescriber (V300) to provide prescribing services for patients of the Clinic in relation to Testosterone Replacement Therapy (TRT), associated treatments, and related hormone therapies.
2. Prescriber Status
The Prescriber confirms that she:
* Holds current NMC registration with no restrictions on practice
* Holds a valid V300 Independent/Supplementary Prescribing qualification
* Will maintain professional registration and prescribing competence at all times
3. Scope of Practice
The Prescriber will provide clinical review and prescribing for:
* Testosterone Replacement Therapy (TRT)
* Human Chorionic Gonadotropin (HCG)
* Aromatase inhibitors where clinically indicated
* Sildenafil for erectile dysfunction where clinically appropriate
* Hormone Replacement Therapy (HRT) where clinically appropriate
* Interpretation of blood tests relevant to hormone and metabolic management
* Ongoing monitoring of patients under TRT/HRT care
4. Clinical Autonomy
All prescribing decisions remain the sole clinical responsibility of the Prescriber.
The Clinic provides the patient pathway, systems, laboratory data, and protocols to support safe prescribing.
5. Governance and Protocol Alignment
The Clinic confirms that it has provided the Prescriber with access to its clinical governance framework, policies, and procedures, including (but not limited to) safeguarding, patient safety, data protection, complaints handling, and clinical protocols relevant to TRT and hormone management.
The Prescriber confirms that she has been given the opportunity to review these documents and has satisfied herself that they are appropriate for safe private practice.
Both parties acknowledge that this mutual review ensures alignment of governance standards.
6. Indemnity Insurance
The Prescriber agrees to maintain appropriate professional indemnity insurance suitable for independent prescribing in private practice and to provide evidence of this to the Clinic upon request.
7. Data Protection
The Prescriber agrees to handle all patient data in accordance with GDPR and Clinic data protection policies.
8. Fees
The Prescriber will be paid £25 per patient review.
9. Termination
Either party may terminate this agreement with 30 days’ written notice.
10. Independent Contractor
The Prescriber acts as an independent contractor and not an employee of the Clinic.
