1. Purpose
What this agreement covers
This agreement sets out the terms under which Youth Revisited Ltd provides TRT monitoring, clinical oversight, quarterly blood testing, and — where selected — medication supply. This subscription does not include TRT medication unless the Monitoring + Medication tier has been selected. This subscription is not active until countersigned by the Provider following clinical eligibility review.
2. Subscription & term
Monthly rolling — no minimum commitment
By signing this agreement the Client requests that the subscription commences immediately. A cancellation window of 7 days applies from the date of signing, provided no Direct Debit payment has been taken and no medication has been ordered. After this period, or once the first payment is collected, the subscription terms apply in full. The Monitoring Only subscription is billed monthly on a rolling basis. Cancel at any time with 30 days' written notice by email to the Provider.
3. Blood testing
Quarterly panel — every 3 months
A full hormone and health blood panel is carried out every 3 months. The standard panel includes: Total Testosterone, SHBG, LH & FSH, Prolactin, Full Blood Count, Haematocrit, PSA, HbA1c, Liver Function (LFT), and Lipid Profile. Cortisol is added where there is a history of steroid use. The prescriber may request additional tests if clinically indicated. Failure to complete required blood tests may pause clinical oversight and any prescribing.
4. Optional upgrades & specialist investigations

The following optional add-ons are available to all clients:
Biomarker Upgrade — £100 per quarter (ad hoc): Upgrade any quarterly blood test to the full Athlete/extended Biomarker profile, covering a broader range of performance, recovery, and metabolic health markers. Includes a home nurse visit. Selected ad hoc each quarter — not locked in at signing.
Specialist investigations: Investigations beyond the standard quarterly panel that are related to TRT and hormone health are available where clinically appropriate. Cost varies depending on the investigation required. Contact the team for more information before requesting — we will advise on suitability and pricing.
Other clinical queries: If you have a clinical question not covered by your standard monitoring, message the support team and we will advise whether it falls within the subscription or requires an additional service.
5. What you receive
Included in your subscription
Blood testing: Quarterly blood panel (all markers listed in Section 3), carried out at home by a qualified phlebotomist or at our clinic.
Results & clinical review: Following each test, an AI-assisted clinical report is generated and reviewed and signed off by Nemia Pilobello (NMC-registered Independent Prescriber), under the medical oversight of Dr Salman Ahmed (GMC-registered, #7948448). You receive this report with your results.
Quarterly consultation: Every quarter, following your blood test, you have the option to speak directly with Nemia Pilobello or Dr Salman Ahmed to discuss your results, treatment, or any concerns. Your choice, based on your needs. Included at no extra cost.
Additional consultations: Further consultations outside your quarterly review are available on request. Pricing varies — contact the team for details.
Ongoing support: Access to your named personal contact Monday to Friday, 9am–5pm, for questions about your programme, results, or appointments.
Prescriptions: Issued by Nemia Pilobello quarterly when clinically appropriate, following blood result review. Dose adjustments made only when clinically appropriate.
Dashboard: Personal results dashboard showing all your biomarkers over time. Compare quarters, track trends.
Medication: Dispensed by SignatureRx (GPhC registered, reg. 9012267) and posted to your registered address following each prescription issue.
Consumables (injection clients only): Needles, syringes, and a sharps bin are included and sent with each quarterly medication dispatch for clients on Sustanon or Testosterone Cypionate.
Injection training: A private 1:1 Zoom session covering preparation, injection technique, site rotation, and aftercare — provided before your first administration.
6. What is not included
Excluded from this subscription
TRT medication (unless medication tier selected) • Needles, syringes, and sharps bins for Testo Gel clients (consumables are included for injection medication clients only) • HCG, aromatase inhibitors, or other ancillaries not prescribed as part of your protocol • Medical letters, GP correspondence, or external referrals • Specialist investigations unrelated to TRT or hormone health • Emergency or out-of-hours clinical cover • The optional Biomarker upgrade (charged separately at £100 per quarter when selected) • Any specialist investigation beyond the standard panel (pricing on request).
7. Pricing
Monthly cost — monitoring included
The agreed monthly subscription is £95/month (Monitoring Only — monitoring included). All prices include monitoring. Medication costs are non-refundable once dispensed. Prescriptions can only be issued when monitoring is current and blood tests are up to date.
8. Payment
Direct Debit via GoCardless
Payment is collected monthly by Direct Debit via GoCardless. A separate mandate setup link will be sent to you following Provider approval of this agreement. Your subscription will not be activated until the Direct Debit mandate is confirmed. The Provider reserves the right to suspend services if payment fails.
9. Prescribing cycle & cancellation
How cancellation works alongside quarterly prescribing
The Monitoring Only subscription may be cancelled at any time with 30 days' written notice by email to info@youth-revisited.co.uk. Services remain active during the notice period. The 7-day cancellation window from signing (see Section 2) applies before the first Direct Debit payment is taken.
10. Medical suitability & approval
Clinical discretion
This agreement is subject to clinical eligibility review. The Provider may decline to activate, suspend, or discontinue services at any time for clinical safety reasons. Additional tests may be required. The Provider may also cancel if the Client becomes medically unsuitable, behaves abusively, refuses required safety testing, or misuses medication.
11. Client responsibilities
Your obligations
You agree to: provide accurate and complete medical information; complete blood tests when requested; use any prescribed medication responsibly and as directed; inform the Provider of any health changes or concerns; and maintain an active subscription while receiving monitoring or prescription support.
12. Risks & informed consent
Known risks of TRT
Hormone treatment carries potential risks including: changes to blood counts and cardiovascular markers; reduced fertility or infertility; hormonal imbalance; mood changes; polycythaemia; skin reactions; and injection-site issues. By signing this agreement you confirm you understand these risks and consent to ongoing clinical monitoring as the primary safeguard.
13. Liability
Limitation of liability
The Provider is not responsible for outcomes arising from misuse of medication, incomplete or inaccurate medical information provided by the Client, delays in pharmacy delivery, or issues arising from medication sourced outside the Provider's framework. Nothing in this agreement limits the Provider's liability for negligence or any liability that cannot be excluded by law.
14. Data protection
GDPR & your data
All personal and clinical data is processed in compliance with UK GDPR and the Data Protection Act 2018. Data is held securely within the Provider's clinical systems and is not shared with third parties except where required for clinical care (e.g. dispensing pharmacy), payment processing (GoCardless), or regulatory compliance.
Important notes
This agreement is for approved clients only. You must have completed an initial consultation with Youth Revisited and received clinical approval before signing.
This subscription is not active until countersigned by Youth Revisited Ltd. Signing this form does not guarantee activation.
The subscription is active from the date of signing. Your first Direct Debit payment is taken 7 days after signing. You may cancel within this 7-day window with no charge, provided no medication has been ordered. After the first payment is taken, the full subscription terms apply.
For clients on the Monitoring + Medication tier: once a quarterly prescription has been issued and medication dispensed, all three monthly payments for that quarter are committed and remain due regardless of when cancellation notice is given. Cancellation takes effect at the end of the current billing quarter.
Results reports are AI-assisted and reviewed and signed off by Dr Salman Ahmed, GMC-registered prescriber. You have the option to speak directly with Dr Ahmed or a clinical team member each quarter following your blood test — this is included in your subscription.
Needles, syringes, and a sharps bin are included and sent quarterly for clients on Sustanon or Testosterone Cypionate. These consumables are not included for Testo Gel clients.
The Biomarker upgrade (£100/quarter) is entirely optional, chosen ad hoc each quarter when your test is due. It is not a committed recurring charge.

Prescriptions can only be issued when blood tests are current. Missing a required quarterly test may pause prescribing until results are received.
A 14-day cooling-off period applies from the date of this agreement unless blood tests or clinical work have already commenced.
Youth Revisited Ltd is not a replacement for your NHS GP or primary care team. This service provides specialist TRT monitoring and does not constitute general medical care.
