1. Purpose of This Agreement

Youth Revisited operates a growing network of clinic venues, gyms, and private locations across the UK, delivering professional blood testing services through trusted, approved clinicians.

This Agreement sets out the relationship between the Company and the Approved Phlebotomist, who is authorised to represent Youth Revisited by delivering clinics and blood draw services within areas allocated by the Company as part of the Youth Revisited Clinic Network.

The Approved Phlebotomist operates as an independent, self-employed practitioner and is not an employee of the Company.

2. Approved Phlebotomist Status

The Approved Phlebotomist confirms that they:

Are qualified and competent in venepuncture practice

Hold valid professional indemnity and public liability insurance

Operate on a self-employed basis

Are responsible for their own tax, insurance, and regulatory obligations

Understand they are representing Youth Revisited when attending clinics

3. Allocated Operating Area

The Company may allocate clinic activity to the Approved Phlebotomist based on location, demand, and venue requirements.

While this Agreement remains active, the Company will not intentionally appoint another Approved Phlebotomist to routinely service the same venues, provided the Approved Phlebotomist is actively supporting their allocated clinic activity.

This does not constitute ownership of a territory or venues.

4. Clinic Allocation & Cover Support

All clinic bookings, home visits, and venue work arising within the Approved Phlebotomist’s allocated area will be routed to them in the first instance through the Youth Revisited booking pathway.

The Approved Phlebotomist is expected to make reasonable efforts to accept and attend bookings within their allocated area.

Where the Approved Phlebotomist is unable to cover clinic activity due to holidays, illness, availability, or other commitments, they agree to provide reasonable notice to the Company wherever possible.

In such circumstances, Youth Revisited may allocate alternative Approved or Sessional Phlebotomists to ensure continuity of service to venues and clients.

This support arrangement forms part of the Youth Revisited Clinic Support Network and ensures that venues and clients are never left without service.

All bookings within the allocated area are managed exclusively through the Youth Revisited booking system.

5. Exclusivity

Because the Approved Phlebotomist represents Youth Revisited publicly at venues and clinics, the Approved Phlebotomist agrees that, while this Agreement is active, they will not:

Provide blood draw services for any other blood testing company, laboratory provider, or testing brand

Approach venues to offer blood testing services on behalf of any other provider

Divert clients, venues, or bookings away from Youth Revisited

Promote alternative blood testing providers to venues or clients introduced through Youth Revisited

If the Approved Phlebotomist wishes to work with other testing providers, they may do so by giving notice to terminate this Agreement.

6. Earnings

The Approved Phlebotomist may earn through:

Paid clinic sessions at Youth Revisited venues

Paid home or workplace visits allocated within their area

A Venue Introduction Fee where applicable

Rates are set out in Appendix A – Approved Phlebotomist Rate Schedule.

7. Venue Introductions & Venue Introduction Fee

Approved Phlebotomists are encouraged to introduce suitable gyms and venues to expand the Youth Revisited Clinic Network.

The Venue Introduction Fee of £50 becomes payable only once the introduced venue has generated £500 in completed sales through Youth Revisited.

Sales must be fully completed, paid for, and fulfilled through Youth Revisited before eligibility is met.

Youth Revisited retains sole discretion to verify qualifying sales before payment is released.

This payment is made once per venue and does not create any ownership, rights, or ongoing interest in the venue.

8. Professional Standards

The Approved Phlebotomist agrees to:

Attend clinics reliably and punctually

Present themselves professionally at all times

Use approved materials, branding, and processes

Deliver a high standard of client care

Represent Youth Revisited positively

Failure to maintain these standards may result in review or termination.

9. Support Provided by the Company

Youth Revisited will provide:

Brand presence and credibility

Website listings and clinic promotion

Booking pathways and client routing

Point of sale materials and literature

Access to the Approved Phlebotomist Resource Hub

Ongoing operational support

Optional supply of kits, consumables, and uniforms

10. Non-Circumvention

The Approved Phlebotomist agrees not to offer private blood testing services directly to venues or clients introduced through Youth Revisited or replicate this model independently using those relationships.

11. Confidentiality

All materials, pricing, processes, systems, and documents remain confidential and must not be shared with third parties.

12. Post-Termination Restrictions (24 Months)

For 24 months after termination, the Approved Phlebotomist agrees they will not, without written permission:

Provide blood testing services to any venue introduced through Youth Revisited

Provide services directly to any client met through Youth Revisited clinics

Replicate this clinic model using those relationships

Promote alternative testing providers to those venues or clients

This applies only to venues and clients introduced through Youth Revisited.

13. Termination

Either party may terminate this Agreement with 30 days written notice.

Immediate termination may occur if exclusivity or professional standards are breached.

14. Nature of Relationship

Nothing in this Agreement constitutes employment, partnership, or franchise.

The Approved Phlebotomist is an independent, self-employed clinician authorised to deliver Youth Revisited clinics.
