The answer usually lies in a single misunderstanding: serum iron and ferritin are not the same thing, and neither one alone tells you your iron status. Iron is one of the most commonly misread markers in routine bloodwork, and getting it wrong means missing deficiency in people who are quietly running on empty, or overlooking overload in people whose bodies are storing too much.

Think of it this way: serum iron is the cash in your wallet. Ferritin is the balance in your savings account. You can have plenty of cash on hand today and still be financially depleted overall — or a healthy savings buffer while your wallet happens to be empty this afternoon.
For most people, ferritin is the more clinically meaningful number. It reflects the underlying trend, not the daily weather. Low ferritin is often the earliest sign of iron deficiency, appearing months before serum iron drops or haemoglobin falls low enough to trigger a formal anaemia diagnosis.
Why the two numbers disagree
Here are the scenarios that trip people up most often — and why relying on a single value causes them to be missed.
Scenario 1: Normal iron, low ferritin
This is the classic case of early iron deficiency without anaemia. Your body is drawing on stored iron to keep circulating levels stable, but the reserves are running down. Symptoms — fatigue, hair shedding, brain fog, restless legs, breathlessness on exertion — often appear at this stage, long before a standard GP panel flags a problem.
A serum iron result in the middle of the reference range can look perfectly fine while ferritin sits at 15 µg/L. On paper, iron is “normal.” In reality, the tank is nearly empty.
Scenario 2: Normal ferritin, low iron
Less common, but revealing. This pattern often signals inflammation rather than a true iron problem. Ferritin is what’s known as an acute-phase reactant — it rises in response to infection, chronic inflammation, liver stress, or even intense exercise in the days before a test. A “normal” ferritin in someone with underlying inflammation may actually be masking depleted stores.
This is why a good clinician will interpret ferritin alongside inflammatory markers like CRP. A ferritin of 80 µg/L in someone with a CRP of 12 is a very different picture from a ferritin of 80 in someone with a CRP of under 1.
Scenario 3: High ferritin
Elevated ferritin is often assumed to mean iron overload — but the majority of raised ferritin results are driven by inflammation, fatty liver, alcohol, metabolic syndrome, or recent infection, not by true iron excess. Genuine overload (as in haemochromatosis) requires additional markers to diagnose properly, particularly transferrin saturation.
Without those additional markers, a high ferritin is just a flag — not a diagnosis.
The full iron panel: what you actually need
A meaningful assessment of iron status uses several markers together. Each one covers a blind spot of the others:
- Ferritin — iron storage; the long-term view.
- Serum iron — circulating iron; the snapshot.
- Transferrin or Total Iron Binding Capacity (TIBC) — the transport capacity available to move iron around the body. Rises when the body is trying to grab more iron from a depleted supply.
- Transferrin saturation (%) — the percentage of transport capacity currently in use. One of the best early markers for both deficiency and overload.
- Haemoglobin and full blood count — shows whether deficiency has progressed to affect red blood cell production.
- CRP (C-reactive protein) — inflammatory marker used to correctly interpret ferritin.
Read together, these tell a coherent story. Read individually, they mislead. A low ferritin with high transferrin and low transferrin saturation is unambiguous iron deficiency. A high ferritin with a normal transferrin saturation and elevated CRP is almost certainly inflammation, not overload. A single number in isolation cannot make those distinctions.
“Normal” range vs optimal range
Here’s the other piece most people never hear from a standard GP result.
The reference ranges on your NHS report reflect the statistical spread of the testing population — not the level associated with feeling well or functioning at your best. Ferritin is the most striking example. UK laboratory reference ranges for women often start as low as 10–15 µg/L, meaning a result of 18 µg/L is reported as “normal.”
Clinical and functional medicine literature increasingly suggests that most adults feel meaningfully better when ferritin sits somewhere in the region of 50–100 µg/L, with athletes and menstruating women often needing to be at the higher end. A result of 18 is technically within range and functionally depleted — which is why so many people are told their iron is “fine” while feeling anything but.
This distinction between statistically normal and personally optimal is where private, interpreted testing earns its place.
Symptoms that warrant a full iron panel
Consider a comprehensive iron panel — not just a single ferritin or iron result — if you’re experiencing:
- Persistent fatigue that doesn’t resolve with sleep
- Hair thinning or increased shedding
- Brittle, spooned, or ridged nails
- Breathlessness on mild exertion or on stairs
- Pale skin, particularly inside the lower eyelid
- Restless legs, especially at night
- Cold hands and feet disproportionate to the environment
- Poor exercise recovery or dropping performance
- Heavy menstrual periods
- Cravings for ice, chalk, or other non-food substances (pica)
- Brain fog, poor concentration, or low mood without obvious cause
Iron demands are elevated in pregnancy, endurance athletes, adolescents, plant-based eaters, and anyone with heavy menstruation, gastrointestinal conditions, or a history of gastric surgery. If any of these apply, single-marker testing is particularly risky.
What to do with your results
If you already have results in hand, the most useful next step is to look at ferritin, transferrin saturation, and CRP side by side rather than fixing on any one figure. If ferritin is low and CRP is normal, deficiency is the likely story. If ferritin is high, transferrin saturation should confirm or rule out true overload. If serum iron is low but ferritin looks fine, an inflammatory picture deserves attention before any iron supplementation is considered.
Supplementing iron without a full panel is a genuinely bad idea. In someone with inflammation-driven ferritin elevation, added iron can fuel oxidative stress rather than correct any real shortfall. Testing first — properly — is not a luxury; it’s the difference between treating a problem and creating one.
